Form 1583, Application for Delivery of Mail Through Agent, is the USPS form behind every virtual mailbox, private mailbox, and mail-forwarding account. It authorizes a commercial mail receiving agency to accept mail addressed to you. USPS requires the form to be verified in person at the mail center or notarized, which is why most virtual mailbox providers tell new customers to get it notarized online.
1. Book a session and upload your completed, unsigned Form 1583.
2. Join the video call at your appointment time. The notary verifies your photo ID on camera.
3. Sign electronically while the notary watches, and the notary completes the notarization.
4. Download the notarized form and submit it to your mailbox provider.
