What Form DS-3053 is

Both parents or guardians must consent when a child under 16 applies for a U.S. passport. If one parent cannot be at the appointment in person, that parent completes DS-3053 to give consent in writing. The State Department requires the form to be signed in front of a notary and submitted with a photocopy of the front and back of the ID shown to the notary.
